This form is required to prove the location is zoned to allow a used motor vehicle dealer license.

Used Motor Vehicle Dealer Application for Address Change
Ohio Dealers must complete this form for any address changes to your dealership.

Dealer Assigment Form BMV 3772
When you become an Ohio dealer you will complete this form on all transactions. It includes what is called "Power of Attorney" which allows you to apply for a new title in your customers name. We will complete an Ohio Dealer Assignment Form in your dealer class.

Application for Ohio Salespersons License
Each person you hire to sell must complete this application. We will complete a sample in the dealer class.

Ohio Dealers Listing of Licensed Salespersons
State law requires a list of all salespersons to stored at the dealership. You must produce this list for law enforcement or any customer that requests it.

Ohio Salesperson's License Termination Notice
You must notify the Ohio BMV when you terminate a salesperson. You will learn the process in your dealer class.

Temporary Tag Void Form
If you make a mistake on a temporary tag state law requires you to complete this form.

I-9 Employment Eligibilty Form
This form must be completed for each partner and employee to ensure they are eligible to work in the US.

Title Gateway Participant Agreement
This form is required to be completed for dealers that want to order titles and pay sales tax collected through the Ohio Title Gateway. You must complete it at a BMV Title Office to get started.
